Doncaster Mental Health Project - On Track

What support is available?

The Doncaster On Track project provides support to service users with mental health needs who need assistance with their home and with establishing independent lives.

The project is the result of close partnership between three key stakeholders: Action Housing, South Yorkshire Housing and Rethink.

 

The needs of each client are discussed with their personal support worker after the initial referral.

We then work in partnership with clients to create a support plan tailored to each individual, which is reviewed regularly.

Areas of support may include:

  • Advice, advocacy and liaison with statutory and voluntary agencies
  • Accessing housing and welfare benefits
  • Budgeting, bill paying and debt advice
  • Access to healthcare and drug/alcohol treatment
  • Maintaining stable accommodation
  • Emotional support
  • Socialising, confidence building and life skills
  • Assistance with finding employment.

Can it help you?

You may be able to benefit from this project if:

  • You are 18 or over
  • You live in the Doncaster area
  • You have a history of mental health problems
  • You have had difficulties in the past with managing a tenancy
  • You have never held a tenancy before and would need support to achieve this
  • You want to work with Action to make positive changes in your life.

How can the support be accessed?

To access the support programme, service users must be referred by a health or social care professional. Some of the agencies that can make referrals to the project are:

  • Community Mental Health Teams
  • Local Authorities
  • GPO and the Assertive Outreach team psychiatrists
  • Social Services.

Where possible, once a referral is recieved, the individual concerned is invited to an assessment to discuss the details of their referral and establish the level of support they require.
 

Service User Involvement:

The Doncaster Mental Health project has an established Service User Involvement Group that meets from 11am to 1pm on the last Thursday of every month at Action's Duke St office. All service users are invited to join in and have their say, or else ask their keyworker to pass any suggestions on for discussion.
